Booking a NEW Patient Initial Evaluation

​
There are several ways to book an appointment. You may call and leave a message at 651-401-2533
and we will return your call within 24 business hours; you may complete our contact form on our website, and we will respond within 24 business hours; you can email us at info@redefinemhw.com and we will respond in 24 business hours, or for immediate scheduling you can follow the instructions below to book online 24/7.

  1. ​Click the green BOOK NOW button anywhere on our website.

  2. Choose your location; Click on in-person or telehealth/virtual visits.

  3. Choose your appointment. You will click NEW Patient Initial Evaluation, or NEW Patient ADHD Evaluation by clicking the orange Book Now button. Please note that ADHD evaluations are only available in-person. 

  4. Select the date/time that works best for you. This calendar always reflects our most up to date availability. 

  5. Enter the client information. If this visit is for a child or other family member, please enter their information, not your own. This information is for the person being seen. Parents may enter their email and phone number for contact information. When finished, click Continue.

  6. Click Confirm Request and your appointment is scheduled!

    When you come for your appointment, please bring your insurance card, your ID, and a credit/debit card or check for any copays, coinsurance or deductibles. 

    Please check your email for the following:

    1. Confirmation email. This email contains information about your appointment date, time, and location. If this is a virtual visit, this email contains your passcode for logging into the visit. I will send this again immediately before your appointment.​

    2. Intake forms. You will receive an email that states, "Please fill out this form." This information is very important in gathering information about you (the patient) regarding physical and mental health history, demographic information (address, phone, emergency contacts, etc.), insurance information, and the necessary consent/legal forms for us to provide care. This information MUST be completed prior to your appointment. If you do not complete this information, we may have to reschedule your appointment.
