How to book online

The booking calendar always reflects our current availability, so whatever you see is genuinely open. You're choosing a New Patient Initial Evaluation.

  1. Click Book Now anywhere on this site.
  2. Choose in-person or telehealth.
  3. Choose your provider.
  4. Choose your appointment type.
  5. Pick a date and time that works for you.
  6. Enter the client's information — see the note below if you're booking for a child.
  7. Click Confirm Request. You're scheduled.

Booking for a child or family member? Enter their information, not yours — the form is for the person being seen. Parents can use their own email and phone for the contact fields.

A note on providers. Dr. Karena Rother sees patients for medication management, ADHD assessment, and TMS. She is not currently taking new therapy-only patients — those appointments are with Bernie Menge or Kelly Brasher. ADHD evaluations are in-person only, with Dr. Rother.

What to bring

Insurance card

The physical card, or a clear photo of both sides.

Photo ID

Driver's license or any government-issued ID.

A way to pay

Credit or debit card, or a check, for any copay, coinsurance, or deductible.

Check your email

After you book, two emails arrive. Both matter.

  1. Your confirmation. Date, time, and location. If your visit is virtual, this email also carries the passcode you'll use to log in — keep it.
  2. Your intake forms. The subject line reads "Please fill out this form." These collect your health history, contact details, insurance information, and the consent forms we're required to have before we can provide care.

The intake forms must be completed before your appointment. If they aren't, we may have to reschedule you — and that's a delay nobody wants. They take about fifteen minutes. Do them the day you book, while you're still thinking about it.
